1.Boy was riding his bicycle when he was hit by a car. After examination in A and E, he was found to have left upper quadrant tenderness. ( Splenic rupture )
2.Man fell off a ladder, has diffuse pain on right side of the trunk. IVU shows no excretion on right side. ( Renal pedicle avulsion ), the fact that no excretion could be demonstrated is pointing to wards pedicle avulsion, mere renal contusion would show hematuria.
3.Man was stabbed with a 10cm knife in the left upper quadrant, stable for 20min after which BP falls to 90/60mmHg.On USG, no free fluid seen in abdomen. ( Splenic rupture ), ideally speaking the closest DD is cardiac tamponade yet in that there should have been Beck’s triad right from the start. The fact that vitals started deteriorating after 20 minutes makes us think on the lines of splenic injury , widely known for haemodynamic instability.

1. A 7 year old has right sided ear ache and rhinorrhea .On examination his right sided tympanic membrane appears pink in colour. AMOXY
2. A 4 year old with right sided ear ache is examined using otoscopy. This reveals a red bulging tym[panic membrane AMOXY
3. An 8 year old boy who has been playing outside now complains of right sided ear ache. On inspection a green bead is seen protruding into his ear canal. The walls of the canal are red. A
4. A 9 year old girl with a history of recurrent otitis media now complains of left sided ear ache. On examination the right sided tympanic membrane appears scarred. On examination of the left ear, pus is seen throughout the ear canal H
5. An 8 year old is noted to have a discharge from the left ear. He has only mild irritation and no significant ear ache. On examination his external canal is inflamed but the tympanic membrane appears normal. H
