PLAB Part 2 OSCE Exam - My Experience

PLAB Part 2

Exam on 7th march 2005 at morning.

Took 30 minutes to reach by Hammersmith line from bromley-by-bow to great Portland street station. They told us to go by 8:45am. I reached there by 8:00am. They took the registration paper; IELTS score sheet, MBBS certificate and passport to check. Took photo and gave a photo ID. Then put all bags and coats in locker. Showed a video for 10 minutes on how to go about in the stations. There was a map to show arrangement of the stations inside. 2 rest stations. 14 exams. No pilot.

Exam started at 9:40am.

1st station was measurement of BP in a person who is feeling giddy for last few days.

2nd station was history taking of a lady with abdominal pain in A&E. Discuss DD with examiner. History was more like acute cholecystitis, pancreatitis…

3rd station was a lady who had LVF two weeks ago. Discuss a treatment plan with her now.

4th station was Hematuria history taking and suggest management. This was a bit confusing. As patient didn’t give a specific history to stone or cancer or blood thinner or polycystic or bleeding disorder. So suggested we need to investigate more to come to a diagnosis.

5th station was primary survey in a lady who fell from 2 meter height. Wearing collar. Speaking. Examiner kept saying do only survey. Don’t need to say treatment.

6th station was phone conversation with a mother. Who is concerned about her baby having diarrhea with mild dehydration.

7th station was Fentanyl dose calculation. Weight of child 25 kg. Give Fentanyl at 5 mcg/kg/hr. 1 ml of Fentanyl has 0.5mg of medicine. Give at 1ml/hr. calculate how much normal saline needs to be added for 24 hours. Total dose needed for 24 hours= 5x25x24=2400mcg
2400mcg divided by 1000=2.4 mg
0.5mg of Fentanyl is in 1 ml of the ampoule
So 2.4 mg is in 2.4/0.5 = 4.8ml
For 24 hours 24ml of saline+drugs will be going
So amount of saline to be added is 24-4.8=19.2ml

8th station was suturing. Pt anesthetized. Wound is not clean. Give two stitches and explain to the examiner what you are doing.

9th station examination of diabetic foot. A pin, tuning fork and hammer was kept near examiner.

10th station respiratory system examination and teach pt about peak flow meter.

11th station a case of PID. Pt got IV antibiotics. On USG you found hydrosalpingx. Tell her about consequences of this.

12th station breaking bad news of mesothelioma to wife whose husband has been diagnosed. And doctors suspect him to live for 6 months. He had a history of working in asbestos factory.

13th station was a patient brought to A&E by police. He can’t remember his name.
Assess cognitive function

14th station was CPR of 6 year old child. Who is already in pediatric ward.
