You must provide the following for Councils consideration:
Either completed Registration form: Form A: Application to Register as a Dentist in NZ if you
are applying for full registration;
Or completed Registration form: Form A: Application for temporary registration as a Dentist in NZ if you are applying for temporary registration
Details of the subjects studied for your dental degree
The number of hours studied for each subject
An official transcript of grades or marks in all examinations
Certified copies2 of all degrees, diplomas and other qualifications. (If the certificates are not
written in English, then you need to include an official English translation with your application)
Proof of immigration status eg. NZ permanent residence or evidence that you have applied
for permanent residence in New Zealand
Certified copy of the personal details pages of your passport
Any other evidence of your knowledge and experience as a dentist
Any relevant evidence regarding your English language skills
Any evidence of your knowledge of New Zealand Conditions of Practice
Documented evidence of change of name where applicable
Statutory Declaration to be signed at the end of the application form
A curriculum vitae which should include details, where possible, of
all primary and postgraduate qualifications with dates
details of continuing education taken since completion of training with dates
current registration and details of dental registration examinations sat
details and dates of positions held in dental practice and the range of practice
details of published research papers and/or presentations
any other supporting information
The correct fees
You must pay fees in New Zealand dollars. Fees can paid by cheque, bank draft, MasterCard
or Visa. If paying by cheque or bank draft, please make it:
payable to the Dental Council of New Zealand
drawn on the ANZ Bank, Lambton Quay, Wellington
Your application will be acknowledged when it is received.
Please arrange for the following to be sent directly to the Dental Council:
A Certificate of Good Standing (no older than three months) to be sent directly to the Dental
Council from the registration board in every place you have practiced dentistry over the last 7
years
Referees reports should be forwarded to the Dental Council directly from three nominated
referees on the forms provided
Provided all the documentation has been received the Dental Council will consider your application at its next monthly meeting and will advise the outcome after the meeting.
2 A certified copy is a photocopy of the original that has been certified as a true copy. The issuing authority, an official with the legal power such as a Solicitor, Notary of the Public, Justice of the Peace or Commissioner for Oaths may certify a copy. The official should endorse the copy "Certified true copy of oriinal document sighted" and his/her
official seal.
