Firstly let me tell you that I am interested in Pulmonary and Critical Care as well. The only route in Internal Medicine Residency and then fellowship in Pulmonary and Critical Care. No direct fellowships.
If you are interested in some hands on experience in Interventional Pulmonology then you should think of it in the US. Many places offer the same. In India there is TIPS (trends in interventional pulmonology) organized by Dr.Pattabhiraman and Dr.Ravi Mehta organized a beautiful hands on workshop in september. The respo0nse was fantastic and the turn out was more than expected. There were animal models as well with bronchoscopy, EBUS, TBNA, Pleuroscopy, APC Cautery, and A rigid station for intubation and a Separate stenting station where Dr.Herve Dutau (from France) took it up... It was an awesome experience.

that will be the "LEAST" and "LAST" thing to affect your chances.
if you got everything right in place... then ur performance in undergrad, doesnt count much. so do not worry about that.
but what all is necessary to keep everything right in place...??
1) US clinical experience
2) research experience
3) fresh graduate
4) give your interview nicely
5) apply to large number of programes
6) apply to img friendly programes and specialities
7) pass the step in FIRST ATTEMPT with good scores.
trust me, if you can do the above mentioned things... you will be able to get residency, atleast in some community programe.

They usually don't see all that... Attempt in your usmle steps matter not in MBBS life

then, USMLE world question bank (online)

i have finished my MD in anesthesiology from india.. now preparing for step 1 since 2 months.. can anyone tell me what would be the best score to get residency in anesthesia. i have heard that its better to do a fellowship program before to improve your CV and then try for residency.is it so?
one more doubt do i require to write steps to join in fellowship too?

its difficult to match in anaesthesiology, NOT IMPOSSIBLE though.
you need to have research experience, some publications and paper presentations. good amount of USCE.

for syep 1 scores:
only 7 people matched out of 40 who were interviewd, with step 1 scores bet 181-190. dats just 17% result. these data are for those who were called for interview, if we actually include all the candidates who applied for the match and did not get interview, the results will be terrible.
they (who got matched with such low scores) surely must be having all round profile.
lets take a look at step 2 scores:
only 2 out of 28 people who were interviewed with scores bet 181-190 were matched. ONLY 7 % RESULT....!!! TERRIBLE
again,
these data are for those who were called for interview, if we actually include all the candidates who applied for the match and did not get interview, the results will be MORE TERRIBLE.
i hope i am clear, not to discourage anyone... getting anaesthesia is difficult and not impossible.
try getting gr8 scores on steps (ATLEAST ABOVE THE AVERAGE SCORES FOR IMGs who were matched)
and remember, only 3 digit scores will matter.... 2 digit score are going away.

and this very fact makes branches like surgery, radiology, anaesthesiology hard to get into for IMGs... its NOT IMPOSSIBLE though.
but i do believe that by presenting the correct picture of the things, i can help sum1 to modify their dreams or to help them work harder for their dreams.
coz USMLE is about a lot of investment (both in terms of money and time)
its always helpful, to "not to be in delusion" because, as a matter of fact, it will not take you anywhere.
but again, by knwing the correct scenario out there for getting matched, may be sum1 will strive HARDER for their dreams,
by knwing the correct scenario may be sum1 will INCREASE their efforts for their dreams, which will actually help you.
ALWAYS REMEMBER THIS:

>>there will be people who will conquer USMLE and get residency AFTER YOU (in the future)

yes, to apply for fellowships, you need to pass USMLE steps. (1, 2 cs and ck)
after doing fellowship in anaesthesiology in US, you cannot work in US. because to work in US, you need to be board certified, and to be board certified, you need to do residency in US.
yes, you can apply for residency in anaesthesia, after doing fellowship in US. then you will have to do your residency in anaesthesia, and get board certified, to be able to work in US.

i understood that it is tough but not impossible to get anesthesia for IMG in USA .By your statistics and advice my goal for doing anesthesia residency has become more strong, if it is tough i have to get it..!!!

As an IMG, you must demonstrate the following to be considered as a candidate for certification by the American Board of Radiology:
You have satisfied the training requirements of a non-U.S., non-Canadian country.
You have documentation verifying your ability to practice radiology independently in your country of training.
You have had one year of clinical training.
You have received board certification in your country (if applicable).
If you have trained in India, beginning January 1, 2012, you must have the following degrees to be eligible for the alternate pathway: M.B.B.S. + D.N.B. and/or M.D. If you have another set of degrees from India, a sponsoring department agreement (SDA) must be received by the ABR by 12/31/2011 in order for you to be eligible for this pathway. After that date, holders of other degrees from India will not be eligible for ABR certification via the alternate pathway.
The board will assess your status relative to ABR policies and alternative pathways toward board certification (see policy) only after receipt of the following documents:
Current, detailed curriculum vitae with specific dates of all training
Verification of:
Medical school training
One year of clinical training
Radiology residency training
Radiology certification (if applicable)
Radiology training appointment and/or full-time faculty/fellowship appointment in an institution with a diagnostic radiology training program approved by the Accreditation Council for Graduate Medical Education (ACGME) or Royal College of Physicians and Surgeons of Canada (RCPSC).
NOTE: The board accepts copies of diplomas or letters from training institutions as verification. If original documents are in a language other than English, English translation must accompany them.
Alternate pathway candidate agreement for diagnostic radiology certification form
Payment of $300 administration fee to be included with the completed application form
Copy of Educational Commission for Foreign Medical Graduates (ECFMG) certificate or Licentiate of the Medical Council of Canada (LMCC) certificate
Sponsoring department agreement, including your four-year plan and a letter of support from the department chair (see sponsoring department agreement)
NOTE: The required four years of full-time, continuous residency training or continuous faculty fellowship experience must be completed in one ACGME-/RCPSC-approved program. Credit for research training of up to 12 months will be granted toward diagnostic radiology certification if the research is completed at the same institution as the remaining clinical training.
Your department chair must complete and submit annual verification forms.
Copies of United States Medical Licensing Examination (USMLE) Steps 1 - 2 score reports
USMLE Step 3 is required six months prior to admittance to oral exam.
Current unrestricted medical license is required before approval to take the qualifying and certifying examinations will be granted.
An IMG must pass both the qualifying and certifying examinations to be certified by the ABR. The rules for conditions and reregistration are the same as for U.S. graduates.

In general, candidates seeking ABR certification through the alternate pathway are not able to seek Authorized User (AU) eligibility through the ABR pathway. However, the ABR will consider exceptions to this policy on a case-by-case basis. If you desire AU eligibility, please submit a letter explaining your plan to meet current NRC training and experience requirements. For more information, please read the NRC regulations section of the website.
If applicable, you must submit a plan to meet the MQSA Breast Imaging requirements.

One of my friend finished internship in 2008. He will be finishing MS surgery in india in 2012 april. If he starts writin the steps, will he get a match? i mean, is he considered old grad? is the MS surgery from india taken as experience? Is it even possible to get surgery residency there/ PLz help with this too
