dear ashb,
what is ecfmg certification?i had given my step 1 in may and am awaiting the scores and my step 2 is due in july.when and how do i have to send this certification? |
Psu ECFMG certification is something for which we are appearing for USMLE. Its something like registration with MCI in India. Once we clear all the USMLE steps upto Step 2 CS, we obtain ECFMG certification, which is required to:
1. Apply for match
2. To appear for Step
3. To obtain practicing license in the US.
I'm quoting here preliminary FAQs from ECFMG information booklet, which i suggest u must read. Download it from ECFMG website for free.
*********************************************
ECFMG Certification FAQs
1. What is ECFMG Certification?
Medical schools outside the United States and Canada vary in their educational standards and curricula. The purpose of ECFMG Certification is to assess the readiness of graduates of these schools to enter U.S. residency and fellowship programs that are accredited by the Accreditation Council for Graduate Medical
Education (ACGME). To be certified by ECFMG, you must pass a series of exams; you must also fulfill ECFMG’s medical education credential requirements. These requirements include providing ECFMG with copies of your medical education credentials, which ECFMG will verify directly with your medical school.
2. Why would I want to be certified by ECFMG?
If you are an international medical graduate and wish to enter an CGME-accredited residency or fellowship program in the United States, you must be certified by ECFMG before you can enter the program. You must also be certified by ECFMG if you wish to take Step 3 of the three-step United States Medical Licensing
Examination (USMLE). ECFMG Certification is also one of the requirements to obtain an unrestricted license to practice medicine in the United States.
3. How can I find out if students/graduates of my medical school are eligible for ECFMG Certification?
To be eligible for certification by ECFMG, among other requirements, your medical school must be listed in the International Medical Education Directory (IMED) of the Foundation for Advancement of International Medical Education and Research (FAIMER). If you are a medical school student and you want to apply for
examination, your medical school must be listed in IMED and your medical school’s “Graduation Years” must be listed as “Current,” both at the time you apply for the exam and at the time you take the exam. If you are a medical school graduate, your medical school and graduation year must be listed in IMED to be eligible for
examination and for ECFMG Certification. To verify that you meet these requirements, access IMED on the ECFMG website at Only RxPG members can see links here! Register or Login!
|
4. How do I begin the certification process?
The certification process begins when you submit your first exam application to ECFMG. At this time, ECFMG will assign you an identification number that you will use throughout the certification process.
5. How long does the certification process take?
The time required to complete the certification process is different for each applicant. You may apply for the required exams as soon as you meet the eligibility requirements for examination. All of the exams required for
ECFMG Certification are offered continuously throughout the year.
Both medical school students and graduates may begin the certification process. However, since one of the requirements for ECFMG Certification is that your medical education credentials be verified by ECFMG with your medical school, you cannot complete the certification process until you graduate. The time required for
some aspects of the certification process, such as the time required by your medical school to verify your medical education credentials, is beyond the control of ECFMG.
6. In what order do I take the exams required for ECFMG Certification?
To be eligible for ECFMG Certification, you must, among other requirements, pass USMLE Step 1, Step 2 CK, and Step 2 CS. See Examination Requirements on page 15. Once you meet the eligibility requirements for examination, you can take these exams in any order.
7. How can I have ECFMG confirm to a medical licensing authority or residency/fellowship program that I am ECFMG-certified?
ECFMG will confirm your certification status when it receives a request directly from a medical licensing authority, residency/fellowship program official, or other appropriate organization. For status reports sent to medical licensing authorities, the request can also be sent to ECFMG by you. Approximately two weeks are
required for the confirmation to be completed. Please note that there may be a fee for this service. See Confirmation of ECFMG Certification Status on page 9. If you apply to residency programs through ERAS, ECFMG will send an electronic ECFMG Status Report automatically to all of the programs to which you applied.

Well i never thought u had asked this.
Waise i'm sorry i used d wrong words.. U dnt need ECFMG certification to appear for match but to Join a residency program!
U have to take Step 2 CS before Dec 31, 2006 to be eligible for 2007 match. U need to complete the ECFMG certification process before u can join ur residency program.
But it wud be better to take Step 2 CS as early as possible as it has its own benefits during match.

I don't think that should be a problem, but since i have never heard anybody else do that before, i'd suggest simply call the ECFMG people once. They are said to be pretty co-operative. At least no harm in trying.

Yes, there is no problem in taking ur spouse along with u, be it d J1 or H1.
Well H1 is definitely better but since, it is offered by few programs, it limits d number of programs u can apply for. It has d advantage that the mandatory requirement of coming back to home country for 2 years (as in J1) is not a headache.
The average FY1 annual compensation for most residencies is about $39,000. So its rarely 4000$. But what i've been told is that its more than enough to make a decent living. In fact people r able to save substantial amounts from this stipend.

LOR is Letter of Recommendation, which states what all qualities d doctor found in u as a medical student. Undoubtedly US LORs are much more valued than Indian LORs, but everybody can't get them.
About d number, yes about 4 LORs from d dept u aim for is good and in general the more the better..

Of course.. usually the only source of LORs for Indian graduates is their own college. If u have enough LORs from US docs, than it won't be that important i think. Otherwise i think, there's no harm in doing that, even if u have LOR from other sources (like frm where u did JRship, PG etc)

Yeah.. as discussed in couple of posts above, about 3-4 LORs from ur dept of interest helps plus u can get 1 LOR each from other major clinical specialities also but not necessary.
N remember US LOR is much more valued than Indian LORs cos they know how r they given out in India.

Dear AshB,
I've gone through your very informative post. There's something that Í'm apprehensive about in regard with the USMLE. The USMLE has a lot of limiting factors that are not in our control, like the visa for CS, getting US clinical experience and LOR's (especially if you have no contacts in the US). From what I've gathered getting into a university program without a USCE or US LOR's is almost impossible. Then the only chance that we are left with are the county programs and the community programs, the draw back in these programs are that getting into a fellowship after these are not heard of since all the fellowhip programs ar university based. And getting a board certification after the county/community programs is very difficult in comparision to the University programs.
I might be totally wrong on this (I hope I'm). Please do help as it would really help me in taking a huge decision between the MLE and AIPGME.
Thaning you in advance

Hi Dr A, U r partly right. See, its certainly more rewarding to get USCE n US LORs but to say its impossible to get good programs without them, is wrong. People like us have gotten into good programs without USCE or LORs too. Good scores n good interview skills matter a lot.
And people from community programs do get fellowhips, it just that % is a bit low. Its not drastically different from uiversity based. Plus u have to try a bit harder if u r from community programs.
Well about board certification, never heard if it was difficult for community program residents.. U r d first one to tell me that. N i think it can't be that way. Though more info is welcome. Do tell us, what gave u this impression.
Acco to me, the pros n cons of Community based programs are:
Pros:
1. Good patient inflow
2. Clinical skills at times better that university based ones
3. Getting into them is easier
Cons:
1. Getting into fellowships is tougher not impossible
2. Academics r said to be inferior
But don't get disheartened JUST DUE TO THIS.. Good luck

About 21% of anaesthesia residents are IMGs. It used to be easy to get into anaesthesia earler but its getting more competitive by d passing year. Still its much easier to get than d likes of general surgery.
Regarding a year in int medicine, i don't have much idea. All i know is that its known as Clinical Base Year or d preliminary year. Some programs have a 4 year course which includes CBY while some programs need u to do CBY before u r eligible to apply.
I got this info somewhere:
"Anesthesiology residency training programs are divided between "categorical" and "advanced" programs. Categorical programs give you all four years of training (PGY1-4), including the preliminary year. Advanced programs provide only the last three years (PGY2-4), so if you apply to these programs, you will have to apply and interview separately for PGY1 internship programs."
Sorry.. No more info..

Hi bergkamp and rajat! Welcome to d world of USMLE.
I'll try to answer ur questions serially..
1. If u read d first page properly total time's mentioned there.. About 14-18 months on an avg.
2. Ophthalmology residency is very competitive, even more than general surgery. U need preferably all 3 -- high scores, some research publication, observerships, to have a fair chance. Risk is pretty high cos even if u do get a slot it myt be a low ranked program. But if u r committed n confident enough its not impossible.
3. See post # 151 on page 15 of this thread for deadlines to apply. An application usually takes 20-30 days to process from d day u send to ECFMG to receipt of scheduling permit, sometimes even more than that. Once u receive d scheduling permit, u can take a date, when n if available.
4. Scores? well if u want ophthalmology, target nothing less that 99 and best 3 digit score possible beyond that.

THANKS A MILLION AshB!!!
That was a very encouraging post.
Some of my friends who are already past their community programs told me about the general trend of fellowships being given to University program people since the fellowships themselves are University based and they more or less have their own residents filling up the post. And the same is the case with board certification, seems the community and county based programs are considered inferior making the board certification a lot lot tougher if not impossible.

Hey vero sorry for being late in my reply..
If u mean MBBS STUDENT i dunno but if u meant an Medical graduate from india who has completed MBBS, surely such a person can do research in US. It depends on the Principal Investigator if he wishes to allow u to work. But its pretty difficult cos visa issues r involved, plus u gotta have some good contacts and do lots of e-mailing and networking.
There is no streamlined process.. U can get into research oriented subjects like pharma, micro, immuno etc via GRE. If u know sum1 or can pull sum strings and can get some prof there to let u to work under his research project, it myt work.
N thanks for ur kind words..

Aim USMLE Step 1

1. For starting residency in 2007, its necessary to have ecgmg certification (need step1,2,cs scores)by 1st september 2006 or atleast score of one of the steps to fetch interviews
2.Yes you have ample time to appear for step1 in november but keep in mind that u'll be eligible for 2008 match.
3.usualy it takes 4 weeks for results to arrive
4.well finish what? only step1 or both?
