1) disorganised/hebephrenic type :
incongrous grinning and grimacing are common in these patients, whose behavior is best described as silly or fatuous.
ref: kaplan and sadock's synopsis of Psychiatry , 9th ed, pg 486

2) tricky question
i would go for valproate for the time being.
if u want to quote synopsis-- carbamazepine or valproate. he does not specify one.

current understanding: valproate,carbamazepine and lithium.
overall evidence for carbamazepine is less than for others.
efficacy of lithium still debated. recent studies show lithium and lamotrigine are quite effective in rapid cycling. keep a watch. u might find a new question with lamotrigine as an alternative too!!!!!
