as AF BEGETS AF , SO DOES CCF.

ONCE IN CCF ,THERE IS MORE FURTHER RECURRENCES (ESP IF THERE IS UNDERLYING STRUCTURAL DISEASE AND LOW EJECTION FRACTION)

MOST OF THE TIME CCF IS PRECIPITED BY PNEMONIA, LOWER RESPIRATORY INFECTION(VERY DIFFICULT TO EXCLUDE INFECTION FROM CCF) AND NON-COMPLIANCE, NSAIDS AND RENAL FAILURE(CAN BE BOTH CAUSE AND CONSEQUENCE OF CCF).

SO THE ANSWER MIGHT BE REFRACTORY CCF ITSELF (PULM EDEMA,CARDIOGENIC SHOCK,CARDIORESP ARREST) OR RENAL FAILURE DUE TO RENAL SHUTDOWN OR DUE TO PNEMONIA/INFECTION WHICH PRECIPITATED THE EVENT OR COMBINATON OF ALL(MODS-DEATH)

since the question says common cause of death...

it should be renal failure which will lead to anuria, azotemia and subsequently death!!

I too agree with you all.........
even though pneumonias are common thigs complication a compensated heart, they too finally lead to:
shock (in septic or vascular)
|
MODS
(renal shut down is a part of it)
|
DEATH
