I would really appreciate your comment about the guidelines for the Anticoagulation.
As a starter i know the general recommendations varies from Hospitals to Hospitals as well as Physicians.
We do the baseline test, FBC, including platelet count PT/INR, APTT, LFT.
Then we start IV heparin therapy and continue until a therapeutic INR has been achieved with PO Warfarin on two consecutive days.

Daily Platelet should be monitored after day 5 of any heparin therapy.( This is to detect the complication of heparin-induced thrombocytopenia)
