in this case suprapubic catheterisation is done& ascending urethrogram(ioc) or flexible cystoscopy done 2 see extent of injury

I agree.
Standard management of all perineal injuries with retention is INITIALLY conservative and urine is siphoned off with the help of either a large bore abbocath or a catheter suprapubically. Many such injuries cause retention due to oedema which subsides in due course of time and in many cases there is partial tear of the urethra which can be negotiated during URETROSCOPY and a silicone catheter passed to bridge the gap till it heals.
In cases of complete tear, if the gap is large then interval surgery is advocated but if two ends of urethra can be made to meet then early surgery may be gratifying in EXPERT hands (only).
In any case no intervention is undertaken prior to thorough evaluation f the whole urinary tract a urogram antegrade or retrograde is essential .
