Hello guys and gals,

I am unable to understand the Pathology of Emphysema and Bronchitis.

They are destruction of bronchus proximal and distal to terminal bronchioles respectively, but they are also associated with dilatation and fibrosis.

Why is there dilatation?

COPD : a disease state characterised by airflow obstruction due to chronic bronchitis or emphysema.

Chronic bronchitis is characterised by bronchorrhea and cough for 3 months for 2 consecutive years.
Emphysema denotes permanent enlargement of air spaces distal to terminal bronchiole with destruction of their walls without obvious fibrosis.

Etiology : 1 cig. smoking ( m/c cause) 2. Air pollution 3. Familial 4. alpha 1-anti trypsin deficiency.

Pathology : Disease progression leads to 2 presentations --
1. Tar (from cigg.) ---> stimulates macrophage release ---> these macrophages release Elastage which dissolves 'elastin' and forms bullae ---> this situation is called Emphysema.Type -1 respiratory failure is present.

2. Macrophage engulfs tar but can't digest it and in long run when low grade bacteria sneek into body ----> since macropahages are occupied,neutrophils come into play---> Death of neutropils and Pus formation---> chronic bronchitis.This type causes type -2 respiratory failure.

Sorry sorry sorry, please forgive me for my absent mindedness, actually i am preparing for my Surgical Graduation Finals.

What i meant was: What is the difference between Bronchiestasis and Emphysema? Now i understand its [bleep] formation due to elastin destruction.
