Shirodkar procedure for the treatment of cervical incompetence is generally advocated in the 2nd trimester. But not after 25 weeks.

shirodhkar operation shld be done around 14 weeks of pregnancy or at least 2 wks earlier than the lowest period of previous wastage as early as 10th wk-dutta 4th ed,pg 183

Ans. 2. At 12 weeks

Incompetent cervix: passive, painless midtrimester cervical dilation results in membrane prolapse and rupture leading to expulsion of immature, nonviable fetus.

Management:cervical cerclage, which involves the placement of circumferencial suture that externally support the internal os.
Cervical cerclage is scheduled for placement between 10-12 weeks' gestation, after sonogram shows a normal viable fetus.This allows the suture to be placed before cervical dilation, effacement and prolapcse of membrane though a dilated cervical os.
