The life cycle of all Plasmodium species is complex.

Infection in humans begins with the bite of an infected female Anopheline mosquito.

Sporozoites released from the salivary glands of the mosquito enter the bloodstream during feeding quickly invade liver cells (hepatocytes).

Sporozoites are cleared from the circulation within 30 minutes.

During the next 14 days in the case of P. falciparum, the liver-stage parasites differentiate and undergo asexual multiplication resulting in tens of thousands of merozoites which burst from the hepatoctye.

Individual merozoites invade red blood cells (erythrocytes) and undergo an additional round of multiplication producing 12- 16 merozoites within a schizont.

The length of this erythrocytic stage of the parasite life cycle depends on the parasite species: 48 hours for P. falciparum, P. vivax, and P. ovale and 72 hours for P. malariae.

The clinical manifestations of malaria, fever and chills, are associated with the synchronous rupture of the infected erythrocyte. T

he released merozoites go on to invade additional erythrocytes. Not all of the merozoites divide into schizonts, some differentiate into sexual forms, male and female gametocytes.

These gametocytes are taken up by a female anophylean mosquito during a blood meal.
Within the mosquito midgut, the male gametocyte undergoes a rapid nuclear division, producing 8 flagellated microgamtes which fertilize the female macrogamete

. The resulting ookinete traverses the mosquito gut wall and encysts on the exterior of the gut wall as a oocyst.

Soon the oocyst ruptures, releasing hundreds of sporozoites into the mosquito body cavity where they eventually migrate to the mosquito salivary gland.

Morphology of the malaria parasite Plasmodium falciparum inside the infected erythrocyte: 1, early throphozoite; 2, early throphozoite (double infection); 3, early throphozoite double chromatin with a few Maurer's dots; 4, late throphozoite with Maurer's dots and crenated red cell; 5, Mature schizont with merozoites and clumped pigment; 6, macrogametocyte with bluish cytoplasm and compact chromatin; 7, microgametocyte with pinkish cytoplasm and dispersed chromatin.

Imp points in malaria

• Malaria – Plasmodium – 4 species
• Vector - ♀ anopheles mosquito
• Only Gametocytes can survive inside the stomach of Mosquito
• Developmental cycle in mosquito → 7 – 20days
• Sporozoites disappear from human blood within ½ hr

• Pl.ovale & vivax → Hypnozoites → Relapse
• Pl.falciparum & malaria → Recrudences

• C/F of malaria manifest during the Erythrocytic phase
• P. falciparum will parasitize any stage of erythrocyte esp young cells
P.S → Only RING FORMS & GAMETOCYTES
• P.vivax & ovale → mainly attack reticulocytes and young erythrocytes,
P.S → ALL STAGES of parasite in RBCs
• P.malaria → attack older cells esp NORMOBLASTS

• Hb F,C & S imparts resistance to P.falciparum.
• P.vivax is resistant to those who lack duffy blood group
• Protection from Falciparum malaria → SCA ,Thalassemia & G6PD deficiency

• P. malariae infection is associated with glomerulonephritis and nephrotic syndrome

• Longest IP – P.malariae [ 35days]
• Shortest IP – P.falciparum [13-14days]

• Schizogony period → Malaria – 72hrs , Vivax, ovale & falcioparum – 48hrs

• Mature gametocytes are ROUND in shape except P.falciparum [ ♀ - cresent shaped / ♂ - banana shaped ]
• Infected RBCs are ENLARGED in P.vivax & ovale

• P.vivax & ovale → SCHUFFNER’S DOTS
• P.falciparum → MAURIER’S DOTS & BASOPHILIC STIPPLING
• P.malaria → ZIEMANN’S DOTS

• Arrangement of merozoites
 Irregular grape-like clusters → Vivax
 Irregular → Ovale
 Regular & grape - like clusters → Falciparum
 Daisy head / rosette pattern → Malaria

• DOUBLE CROMATIN / MULTIPLE RINGS / ACCOLE FORMS → P.falciparum
• AMOEBOID TROPHOZOITES → P.vivax
• BAND FORM TROPHOZOITES → P.malaria
