Anyone can apply for these visas, its just a visitors visa that they issue so one can visit the US (as a visitor, not with the intention of working there).
You dont need to pass any exam to get these visas but obviously you will have to show- why do u wanna go to US, some evidence that you will come back after the visit (wont stay there as an illegal immigrant)

For doctors they issue two visas: J1 and H1B
J1 is issued to a doctor who wishes to get higher training in the US. This visa is issued to a doctor if they think the doctor wants to get the higher (medical) training with an intention to serve their home-country. Thats why they want u to go back (to your home country for at least 2 years) once you finish ur training (post-graduation). There is a way to get around this problem as well (I dont know how it works exactly). Apparently if you agree to work in under-served areas, you might be able to stay in the US. As I said I dont know the details.

H1B visa is issued to highly skilled professionals. A doc can get it provided u pass Step3 and the residency offering university offers the H1B sponsoring programs. If you are on H1B u dont need to come back to your home country.

BUT there might be some changes in the US immigration policy from this year, regarding H1B visas for doctors.
