Something from about neuromyotonia..

It is also known as Isaac's syndrome

It is d/t hyper-excitability of peripheral nerves..

C/F:

Muscle stiffness at rest that persists during sleep..
Delayed relaxation after voluntary effort(pseudomyotonia)..

distal muscles are affected most severely..

stiff gait
continuous muscle fiber contractions cause undulation of overlying skin- (myokymia or fasciculations)
excessive sweating d/t heat generated by continuous muscle fiber activity..
cramps..

Paraneoplastic neuromyotonia is a/w benign & malignant thymoma & Small cell Ca Lung

Lab findings:

EMG: fibrillations; fasciculations; doublet, triplet & multiplet single unit(myokymic) discharges, rarely high frequency neuromyotonic discharges..

Autoantibodies to voltage-gated potassium channels

Rx:

Plasma exchange

Phenytoin & Carbamazepine may also improve symptoms..
