B Fine needle aspiration

D Pituitary magnetic resonance imaging (MRI)
E Radioiodine uptake
F Serum follicle stimulating hormone FSH
G Thoracic inlet x-rays
H Thyroid antibodies
I Thyroid releasing factor (TRF) test (TSH)
J Thyroid stimulating hormone
K Tri-iodothyronine (T3)
L Ultrasound of thyroid

37.A 14-year-old euthyroid (normal serum T3 T4 concentration) girl presents with a soft, diffusely enlarged thyroid gland, which moves freely on swallowing.
38.A 42-year-old woman on thyroxine replacement therapy comes to the outpatient clinic to have her treatment checked.
39A euthyroid (normal serum T3 T4 concentration) 25-year-old woman has a solitary, solid thyroid nodule confirmed on ultrasound scan.
40A 60-year-old man who is euthyroid (normal serum T3T4 concentration) has presents with an asymmetrical thyroid swelling.
41.A 30-year-old woman with normal serum T3T4 concentration has palpitation, tremor and weight loss.

well, i posted this theme some time ago. and i re-posted it on the PLAB 1 forum.
we went throught soem discussions and we finally agreed on a conclusion. the theme wasnt accurate, (sorry for that). im posting the best clarifying post for this theme (by Sal):

I am posting the original theme, because there are a couple of mistakes on the old circulating one, especially with in Q 41

Options

Instruction

For each patient described below, choose the SINGLE most appropriate test to be done next, from the list of options. Each option may be used once, more than once, or not at all.


37. A 14-year-old euthyroid (normal serum T3 T4 concentration) girl presents with a soft, diffusely enlarged thyroid gland, which moves freely on swallowing.

My answers are:

37. C(No test at this stage)
It is more likely a case of a non-toxic enlargement of the thyroid gland, taking in concideration the age of the patient. This is characteristic of a physiological goitre which may occur at puberty or in pregnancy

38. J(Thyroid stimulating hormone)

39. E(Radioiodine uptake)

Please check that information on the OHCM sixth edition on page 516.
It may show a malignant lesion as a cold tumor, whereas a hot lesion will suggest only adenoma.

1. Do T3 & T4

2. Ultrasound

3. Radionucleotide scan

4. FNAC

40. L(Ultrasound)

Here is a symilar sytuation as in Q39. Our patient have got done already T3 and T4, then the next step will be ultrasound to check if the lump is solid or cystic.

41. K(Triodothyrinine T3)

in41,it is triky....i thought t3,t4 both normal....hurried!!

but u can do...tsh ab right?female,middle aged.hope i wont do this mistake again..!
