well, before coming to any conclusion read page no 311 of KDT.depolarizing agents produce dual mechanism neuromuscular blockade (under certain circumstances) which can b divided into two phases
phase 1 block and
phase 2 block ---> is slow in onset and results from desensitization of the receptor to ACh --> it therefore superficially resembles block produced by dTC.
so, the conclusion is dTC has nothing to do with phase 2 block,, it is just that phase 2 block resembles it's mechanism superficially.

u r right in saying that Scoline is a dual blocker, so, out of the options given only scoline can cause phase 2 block.

Well,I am really sorry but please can u tell me what is phase 1 block and phase 2 blockade?what r d characteristics of both types??wht is d difference btwn them??
If you can find them I hope u can understand that succinyl choline/suxamethonium/scoline can be one answer under specific circumstances but not always...or not atleast when d-TC and Vecuronium are given as choices ih phase 2 blockers.....
Please Doc Smita,answer those simple questions and let me know why and how had u included scholine as a phase 2 blocker and not vecuronium or d-TC???

Okay frnds,
I got the answer....I hv searched thro many books and there are some contradictory opinions....
like from A.K.Paul-DRUGS AND EQUIPMENT IN ANAESTHETIC PRACTICE,4 th edition...In chapter 5 ,neuromuscular blocking agents...on page 59 there is classification on muscle relaxants:
A: Pharmacological level of action:
1.Curare like drugs-competitive PHASE 2
BLOCK
2.SUccinyl choline like drugs-non competitive
Phase 1 block

B: Physicological or cellular level of action:
1. Non depolarizing
2. Depolarizing.
Now on page 76,of A.K.Paul it is said that phase 1 block is also called DEPOLARIZING BLOCK,and the second form of block is called PHASE 2 BLOCK,desensitization block or dual block....WHICH RESEMBLES BLOCK PRODUCED BY NONDEPOLARIZING MUSCLE RELAXANTS with its associated characteristcs of tetanic fade,post tetanic facilitation and reversal with anti cholinesterase drugs..
So Here is some contradiction in the last paragraph and first classification...
So the dual block is the phase 2 block which is produced only by SUCCINYL CHOLINE,d-TC and Vecuronium produce Non depolarising block which is mimicked by succinylcholine in large doses...

SECOND AND MOST IMPORTANT REFERENCE IS FROM "MILLER",THE GOD FATHER BOOK IN Anaesthesia :-IT SAYS PHASE 2 BLOCK IS PRODUCED BY SUCCINYLCHOLINE.

so Here the controvercy ends...
I M SORRY Dr.Smita...But I hv to search a lot before saying sorry to U....

hey! no sorries needed. this Nagging forum is meant for healthy discussion. infact discussion makes us open and refer to our books, this way atleast we both will never forget this info about succinylcholine.

Yep phase 2 block is caused by excessive successive doses of scoline.
