yes it would be suicidal ideation. again nothing is absolute.
research trends show that ocd is protective against suicide.
alternatively certain types of obessions - such as sexual, blasphemy,impulses to kill others/dear ones - are highly linked with co- morbid dep and high rates of self harm and suicidal beh
i would be careful associating psychosis with suicide or branding that all suicidal clients are psychotic, that is usually not the case.

normal thought is seen in oc clients because the oc thoughts vary in intensity and frequency.majority of clients are able to distract themself from the distressing thoughts.
i would be hesitant to put normal thoughts as the answer , but it would depend on the wording of the q too.

ANS should be suicidal tendency. Only severe cases may have this. Obsessive thought is kind of overvalued idea (normal) not up to delusional level.

I think normal thougt is right answer b/c acc to Kaplan OCD is under anxiety disorders ,and as obsessions are anxiety provoking intrusive thougths are not normal thoughts,and as in oc neurisis symptoms wax and wane e depression and anxiety so it excludes first 2 options,and last option suicidal tendency too.suicidal tendency is more related to MDE,and here thougts are irrationaland MDE is not common.
