For me the option 4) is correct. I come from clincial trial background so it may be useful.
I will discuss in detail why is it so
see randomizaiton is must for any clinical trial, is n't it? bcoz without randomization there will bias. u can exclude option 2
after randomization is done there should either the study should be blinded or atleast cross over if not blinded, to avoid bias again, so that excludes option 4
now option one and four good ones, they compete but if u look at option it is best bcoz u have first randomization, then u double blinded the study, then u r also doing a cross over that means ( both group of patients getting Reference and Test products). Obviously option 4 will be better.

Though not in the options, the best would be randomization with triple blinding....Here investigator, study subjects and even the data analysts dont know who is getting what.

i agree e younaw ans 4 is correct best option.as for ethical reason in crossover study no gr involved can remain untreated all subjects receive intervention but at diff time ,so i think it will be better to avoid bias that RCT should design with double blind and cross overstudy, beside expensive.
