The most commonly involved extraocular muscles are (in order of frequency from most to least common) the inferior rectus, medial rectus, superior rectus, oblique muscles, and lateral rectus

Well, Kanski says, The single most common ocular motility defect is a unilateral elevator palsy caused by a fibrotic contration of the inferior rectus muscle.. It mimics a superior rectus palsy d/t restriction of upward gaze..

The next most common is a contracture of the medial rectus causing failure of abduction simulating a 6th nerve palsy..

Superior rectus involvement is relatively uncommon causing impairment of downward gaze..

The least commonly involved muscle is the lateral rectus causing failure of adduction..

Well, Khurana also says inferior rectus muscle 1st & medial rectus next..

So? I think we should follow Ophthal books for Grave's ophthalmopathy questions..

As I also heard of inferior oblique & superior oblique in surgery thyroid case.. But now I'd like to say Inferior rectus.

The order of occurence is :
IR, MR, SR and LR.
Interestingly, the same order is followed by BCC (Basal cell CA): Lower lid, Medial Canthus, Upper Lid and Lateral Canthus. [The worst Prognosis is for Medial canthus tumours due to the involvement of the lacrimal system]

I think the best option would be INFERIOR OBLIQUE because there is difficulty in looking upwards and outwards.
