I searched for this in our standard text books, but didn't get the answer.

Question is : Does endopthalmitis involve the sclera ?

I very well know that panopthalmitis invloves and destroyes the sclera, but what about endophthalmitis ?

i involves only d inner layers..so sclera not involved.. else u cannot go in 4 eviseration na..

Yes, good logic, but in fact we perform Frill evisceration, in which sclera is excised leaving only 3 mm frill around the optic nerve.. And according to Khurana, it is indiated in panophthalmitis. Mainstay in endophthalmitis is not evisceration..

Sclera includes episcleral tissue, sclera proper & lamina fusca(the inner most part which blends with suprachoroidal & supraciliary laminae of uveal tract).

According to Kanski,

Endophthalmitis is a severe form of intraocular inflammation involving the ocular cavities & their immediate adjacent structures without extension of the inflammatory process beyond the sclera.

Panophthalmitis is similar to endophthalmitis except that the inflammatory process also involves the outer ocular coats as well as Tenoin's capsule.

So, we cannot deny if lamina fusca is involved in endophthalmitis, indicating involvement of sclera..

endophthalmitis is otherwise called vitreous abcess> so SCLERA is not involved. solves problem
