answer is fracture shaft of humerus.
ref bailey page no 588 .
most common cause of radial n injury is fracture shaft of humerus as the nerve passes thru the radial groove of humerus or as a result of pressure as in saturday night palsy due to a heavy sleep with the arm over the sharp back of a chair.

don't have references but i think it should be # shaft humerus coz of reasons mentioned by smita.

e medicine says-''Nerve injuries are much more common than vascular injuries, especially with anterior or inferior dislocations. The axillary nerve is the nerve injured most often and may be crushed between the humeral head and the axillary border of the scapula or injured by traction from the humeral head. Axillary nerve injury has been reported in as many as 33% of acute anterior dislocations.''

i would go for b.

partha
