apart from pregnancy, listeria causes infections only in immunosupressed
doesnt cause gastroenteritis
CNS infection is nostly merningitis. other invasive CNS infections also occur. meningoencephalitis can occur but not isolated encephalitis only
based on the above single best option would be option A
nowhere is a skin fistula mentioned as a compliation in my knowledge, but pls confirm this last fact yourself

for sure the answer is option a)
listeria can cause gastroenteritis, but is not a major cause of sporadic diarrhoea.
most common cns infection is meningitis, meningoencephalitis can occur , but encephalitis alone not mentioned.
definitely not a cause for malignant fistula.
hence, answer of choice is option a) this is something we are sure of.

Some points are noteworth regarding LISTERIA
1.Gram positive aerobe
2.Grows best at 30-37 degress and is NON MOTILE AT 37 DEGREE..SHOWS TUMBLING MOTILITY AT 20-25 degrees...
3.Grows on ordinary media..but IMPROVED GROWTH IN TRYPTOSE PHOSPATE OR THIOGLYCOLLATE BROTH AT 4 DEGREES----this is called COLD ENRICHMENT
L.monocytogenes is the major species and can be distinguished from other listerias in the following features..
A.B-HAEMOLYSIS OF SHEEP AGAR..
B.FERMENTS GLUCOSE ,MANNOSE BUT NOT D-XYLOSE
Infections caused are
1.in pregnancy-mostly in 3rd trimester..
causes transplacental spread and lead to CHORIOAMNIONITIS,STILL BIRTH...etc
2.NEONATAL LISTERIOSIS
Early type-in the 1ST WEEK OF LIFE...
MANIFEST FROM 2ND DAY ITSELF
Present with SEPSIS,RESP.DISTRESS OR GRANULOMATIS INFANTICUM--A generalized abscess formn in liver,kidney,adrenals etc(*********very important**)
Late type--MOSTLY PRESENT AS MENINGITIS..
infection is acquired from birth canal..
3.IN IMMUNOCOMPROMISED..
Most common presentation is BACTERMIC INFN WITHOUT EVIDENT FOCUS
2ND most common-MENINGITIS
L.monocytogenes is the MOST COMMON CAUSE OF INFECTION IN RENAL TRANSPLANT PATIENTS AFTER 1 MONTH
TREATMENT
Cephalosporins TOTALLY INEFFECTIVE
DOC-IV Ampicillin or pencillin+aminoglycoside combin
if pencillin allergic-COTRIMOXAZOLE
PENCILLIN ALLERGIC 3RD TMSTR PREGNANT-ERYTHROMYCIN
