You are the paediatric senior house officer. Mrs and Mr. Brown have come to your clinic as they wanted to know about MMR vaccine as their son Joshua who is 12 months old is due for MMR vaccine.

Introduction

Hi, Mr. and Mrs. Brown, I gather that you have some concerns about Joshua having the MMR jab. Can you tell me your concerns so that I can give you more information.

What is MMR vaccine?

This is nothing but one of the Jabs we give to all children as part of their vaccination schedule. This is to protect Joshua against three serious infections namely – Mumps, measles and Rubella.

Mumps is the biggest cause of viral meningitis. Complications from measles can kill children or cause permanent brain damage

The vaccine is given usually between 12 and 15 months of age. It is being used safely worldwide for a long time.

Are their any side effects to this vaccine?

Sometimes a week after the injection, the child might get fever and rash which usually resolve in a day or two. Occasionally the children might develop a mild form of mumps (swelling of face), which again gradually disappears in a few days. None of these reactions are serious.

The actual harm caused by these infections far outweighs the risk of giving this vaccine to Joshua

What about MMR causing autism as seen in news and press?

Recently there have been some concerns that the MMR vaccine may cause autism or inflammatory bowel disease but there is no definite proof about this.

Recent large studies have shown that there is no evidence to link MMR to these problems.

Autism was well known long before MMR vaccine was introduced. There was no sudden increase in autism when the vaccine was introduced. Parents often notice the signs of autism around the time of MMR vaccination. This does not mean that one causes the other. Experts from around the world including the World Health Organisation agree that there is no link between MMR vaccine and autism.

Can single vaccines be given?

Yes single vaccines can be given but
This has not been tested
MMR vaccine is the standard worldwide More importantly
Six separate injections have to be given over a long time
There is no protection between the injections
Children may catch the disease from others who are also not protected
Over all the vaccine coverage decreases
If you become pregnant again, you will be at a greater risk of rubella infection
Single vaccines are not available on the NHS
We still do not want to give these vaccine ?

As I have just explained, the MMR vaccine is a safe universally used vaccine which has saved many lives.

There are definite risks attached to either not giving the vaccine or using single vaccines.
But the choice is finally yours. We can give you some information sheets about MMR vaccine and you can always come back to us if you have any more queries.

Why it should be done

Measles, mumps and rubella are diseases with serious complications:

Measles – causes a range of symptoms from ear infection and bronchitis to convulsions (fits) or brain damage, and can be fatal.
Mumps – used to be the main cause of viral meningitis in children. Also causes temporary deafness, miscarriage, inflammation of the pancreas, and pain and swelling in the testicles in older males.
Rubella –can lead to painful joints, blood disorders and/or swelling of the brain (encephalitis).

When is should be done
The first MMR vaccination is given to children at around 13 months of age, with a booster dose given before they start school (usually between 3 and 5 years old). Between 5 and 10% of children are not fully immune after the first dose, so the booster gives increased protection resulting in less than 1% remaining at risk.

How it is performed
Parents or carers of children aged around 13 months and/or at pre-school age will be invited to arrange an appointment with their GP practice for the MMR vaccination.

The vaccine is given as one injection in the upper arm. There may be some redness and swelling of the injection site but this should soon disappear.

Single vaccines for the separate diseases (measles, mumps and rubella) are available at some private clinics. Having single vaccines usually involves scheduling them several weeks or months apart. However, the Government strongly recommends the MMR vaccine to prevent putting children at risk of these serious illnesses while waiting to complete a course of single injections.


Risks
People are not given the MMR vaccine if they:

are very ill,
have already had a severe reaction to MMR in the past,
are pregnant - women should avoid becoming pregnant for 1 month after having the MMR vaccine,
have had an injection of immunoglobulin or another blood product in the previous 3 months,
know that they may react severely to the antibiotic preparations neomycin or kanamycin, or
have an untreated malignant disease or altered immunity.
Very few children are unable to have the MMR vaccine for medical reasons.

Side effects
MMR contains three separate vaccines in one injection. Each vaccine has different side effects at different times:

a. About a week to 10 days after the MMR immunisation some children get a very mild form of measles with a rash, a high temperature, loss of appetite and general feeling of being unwell for about 2 or 3 days.
b. About three to four weeks after the injection a child (1 in 50) can develop a mild form of mumps with swelling of the glands in the cheek, neck or under the jaw. This only lasts a day or two.
c. In the six weeks after MMR a rash of small bruise-like spots may occur. This rash may be caused by the measles or rubella parts of the immunisation. Although rare, this usually gets better on its own. However, as with any rash, it is wise to get a doctor’s advice as soon as possible.
d. Very rarely, children can have severe allergic reactions straight after any immunisation (about 1 in 100,000 immunisations for MMR). If the child is treated quickly, he or she will recover fully. People giving immunisations are trained to deal with allergic reactions to vaccines.
People who have been recently immunised cannot infect others with the viruses contained in the MMR vaccine.

Secondly, the possible concerns of father:

a. side-effects
b. read somewhere abt autism
tell him that though a study has been published, it is still not validated.
c. he has had enough jabs, why expose him to more?

If Mum asks why my child cant have single vaccines?

*Tell her that all evidence shows that the MMR vaccine is the safest way 2 protect ur child against mumps,measles n rubella.

*In the time gaps between the 3 vaccines,children would not b protected against the diseases they had not yet been immunised against.so ther would b an increase in mumps, measles n rubella leadin to illness.also unprotected children can pass on diseases to others at risk children n pregnant women.
*Not evrybody will b able 2 finish the course of 6 injections(initial dose n second dose) and so some children willb left unprotected.
