A 47-year-old head coach collapses during a tournament basketball game and is brought to your emergency room vomiting bright red blood. His blood pressure is 70mmHg systolic and his pulse is 120 beats per minute. His hematocrit (packed cell volume) is 45 percent and his hemoglobin is 15 grams/dL; with a white blood cell count of 8,000, an unremarkable differential white blood cell count, an unremarkable peripheral blood smear, and a platelet count of 300,000. In additional to a bolus of
intravenous saline solution he will need
*A. Transfusion of intravenous white cells
*B. Transfusion of intravenous platelets
*C. Transfusion of intravenous plasma
*D. Transfusion of intravenous packed red cells
*E. Transfusion of intravenous atmospheric air (20% oxygen/ 80% nitrogen)

Hello doctormoony,
do you think that the clue of this qn is that RBC count is not given, and all the others are normal mix our minds, and that`s why we should choose to give packed RBC's?
Whe i saw this qn i got confused!

i actually thought the answer was plasma. He's hypotensive and hypovolemic, with everything else normal. Please explain.

Then which would be the right one? There is no plasma, nor colloids offered!
I still would agree with doctormoony's answer, as RBC's are the only value not given.

tHE PT IS VOMITING BLOOD 'HEMATEMESIS' ACUTELY AND HE HAS HYPOVOLEMIC SHOCK
IN THESE SITUATIONS WE GIVE SALINE TO EXPAND CIRCULATION TILL WE GET HIM CROSS MATCHED FOR RBC TRANSFUSION.
IF THERE IS SEVERE BL LOSS AND U DON'T REPLACE RBC THE PT COULD GET ISCHAEMIA
