What should be sent for the RE-ALLOCATION??

in the prospectus they written...

Candidates should firmly tie or staple together his/her APPLICATION FORM, UNDERTAKING and ENCLOSURE(S), if any. Please read Para 11 (i) carefully and follow the instructions.

not sure about ENCLOSURE(S)... even after reading Para 11(i), can't get what other things should be sent with application and undertaking... Please one who have sent an application elaborate more on this aspect, what other documents should be sent with the application and undertaking?

and the address to sent application is:
Assistant Director General (Medical Education), Medical examination cell, Dte. GHS, Nirman Bhawan, New Delhi-110011.
