yes you need it if you want a quick visa to the usa.
otherwise you will have to apply for the B1/B2 visa which is usually issued for for the step-2 cs takers.
when you have an invitation letter from your sister
and you go for visa donot mention that you also want to take step-2cs,simply say you want to meet your sister.that i think will be enough a reason for them to issue you a visa to the usa.

Hood 123 is right in the sense if you want a B1/B2 visa which is usually isued for for Step-2CS.
In my opinion you have two genuine reason to go to the USA one is Meeting your sister who is a green card holder and another is your USMLE steo-2CS.
You can provide either reason to the embassy officials.
But if you simply fill the VFS ( DHS forms ) for USMLE STEP-2CS ,you can even mention the name of your sister in the form in the space provided for it.This can help in the issuane of visa to you. So, simply suggest the USMLE STEP-2CS as a reason for travelling to the USA.Due to this you can get a 10 year ,multiple entry visa to the USA as well.
Good luck.
