An eldely housewife lost her husband who died suddenly of MI a couple of years ago.About aweek after his death,she heard his voice.Subsequently,she heard his voice conversing with her and she would discuss her daily matter with him.This however provoked sadness of mood and anxiety when she was preoccupied with his thought.What is the diagnosis?What should be the treatment?

if the duration is short it is grief reaction but if it is prolonged it would be ablnormal grief reaction. but detailed history needs to be taken to rule out depression severe with psychotic symptoms. if its abnormal grief reaction counselling would be beneficial.

she started hearing voices first and subsequently was preoccupied with thought which PROVOKED depression.
but time should be there to make it schiz.

I think the ans should be depressive disorder e psychotic features (HALLUCINATION),although schiz is most common cause75% of hallucinations but as HX suggest an elderly women,and her husband's death then hallucinations i would prefer my option,otherwise if we may go towars hallucinations we should notice age of patient,sex of patient also,and if we exclude her husband's death then we might think about delirium first.Rx options :
Antidepressant
Anxiolytics
Antipsychotics
ECT
Light therapy

Well Answer is Schizophrenia..
A close look at the Q indicates that the stressor that triggered the psychosis is a few yrs old..
So the presence of hallucinations for the period more 6 months satisfies both ICD n DSM 4.
I do not think it is Schizo affective.Cos she feels sad when she is preoccupied with thoughts of her husband.This is a appropriate emotional response andnot suggestive of a prolonged mood disorder.
