It is D & E are true. In flail chest there is deformity of the chestwall. sputum retention is not given as an option in any text. inadequate tidal volume is also false

E is not a correct option. there is head injury due to which there is central depression of respiration. so such patients need to be put on a ventilator

indications of tracheostomy :

conditions causing RESPIRATORY OBSTRUCTION
1 infections like ALTB, acute epiglottitis, diphtheria , ludwig's angina, peritonsillar / retropharyngeal / parapharyngeal abscess.

2 trauma-- injury to larynx or trachea / fracture mandible/ maxillogacial injuries

3 neoplasms of LARYNX / PHARYNX / upper trachea/ tongue / thyroid etc.

4 foreign body larynx

5 oedema of larynx such as due to irritant fumes etc

6 b/l abductor paralysis

7 congenital anomalies like laryngeal webs etc

conditions causing RETAINED SECRETIONS

1 inability to cough as in coma of any cause ( head injuries, cva etc)

2 paralysis of respiratiory muscles as in GB syndrome, myasthenia gravis, spinal injuries

3 spasm of respiratory muscles as in tetanus, strychnine poisoning

4 aspiration of pharyngeal secretions as in bulbar polio, polyneuritis etc

conditions causing respiratory insufficiency such as chronic lung conditions as in emphysema, chr bronchitis, etc.

so, option d) tetanus is definitely an indication
option c ) flail chest can be true , since IPPV has to given and if IPPv is required beyond 72 hrs, tracheostomy is superior to intubation.
acute head injury is not an indication , intubation is done in this case.
