at first glance i too searched for 'all of the above'.
Is the ans prolonged bed rest? The bone changes in the others not being 'osteoporosis'?

agree with rk1972. All except Vit A deficinecy. there are studies saying excess of Vit A might cause osteoporosis bc of osteoclatic activity.

causes of secondary Osteoporosis;
1- Immobilization
2- malignancies
3- Renal disease
4- Hyperparathyroidism and other endocrinopathies
5- drugs,
6- rheumatologic disorder

I think the question should be rearranged to "except"
