It is Srivatsav Committee.

Reference from Park 18th edition Pg.695
The Village Health Guide scheme was launched on OCtober 2, 1977( under the Rural Health Scheme 1977) by the Govt. of India, based on the principle of "placing people's health in people's hands". It is a three- tier system of health care based on the recommendation of the Shrivatsav Committee in 1975.
