Endodermal Sinus tumor

Endodermal Sinus Tumor is the answer for the last Q.
ref:
which says that:
| Quote: |
| Chemotherapy is preferable, despite these tumours being highly radiosensitive (except EST and embryonal carcinoma), in order to preserve ovarian function. |
So, EST is not radiosensitive.
Note: Embryonal carcinoma is not the same as Polyembryoma. And just to review the rest:
Embryonal Carcinoma
Gross examination of embryonal carcinoma reveals a solid, haemorrhagic, necrotic tumour, resembling a larger form of EST. Embryonal glands, glandlike clefts (embryoid bodies), and syntrophoblastic giant cells are present microscopically.
Polyembryoma
Histological analysis of polyembryoma demonstrates erythroid bodies in different stages of presomite development.
Endodermal Sinus Tumor (EST)
Gross examination of EST, also known as yolk sac tumour, demonstrate smooth, glistening, hemorrhagic and necrotic surfaces. Histology reveals a wide range of patterns (microcystic, endodermal sinus, solid, alveolar-glandular, papillary, macrocystic, hepatoid, primitive endodermal). The classic pattern contains Schiller-Duval bodies (central capillary surrounded by simple papillae) and eosinophilic globules containing AFP. Intracellular and extracellular hyaline droplets (periodic acid-Schiff positive) are also seen in EST.
They ask about Schiller-Duval bodies quite frequently, and the way I remember is:
Robert Duvall (bald oldish actor in Apocalypse Now, The Godfather)
Robert Duvall has an egg-like head
Eggs have Yolk
Yolk sac tumor has Schiller-Duval bodies
(hee, hee!)

As far as the potential of malignancy, it is rare. In one study of 13,000 specimens excised because of leiomyomata uteri, only 29% were malignant. In a more recent study of patients in the reproductive age group, 800 specimens yielded only a 1% incidence of malignancy. The sarcomatous changes that are associated with malignancy are more common in the submucous tumors. The malignant diagnosis is made at histologic examination, with greater than 10 mitotic figures per high-powered field. If the tumor is noted to have 5 to 9 mitotic figures per high-powered field, the patient has a tumor with a low malignant potential. If patients have had a myomectomy, they might be closely observed after consultation with a gynecologic oncologist. In rare instances, these tumors have been noted to metastasize to the lung and throughout the abdomen.
