A 28 year old man has a family history of sudden cardiac death at a young age. Physical examination shows an ejection systolic murmur that decreases in intensity when the patient lies down and increases in intensity when the patient stands up . An ECHO shows abnormal anterior movement of the mitral leaflet against an asymetrically thickened interventricular septum. What is ur diagosis ???

This is a very easy one......if you can give the reasons for ur answer it wouuld be better...........!

abnormal movt. of anterior valve leaflet... d/t aortic regurgitation.. caused by improper opening n closure of thickened valves..

the answer i expected is Hypertrophic obstructive cardiomyopathy...
Rashmilla u were right in sayin there is an anterior mvmt of the anterior mitral leaflet.....it occurs during systole....In HOCM the septum is thickened and the mitral valve comes in apposition with it in manoeveures which decrease the end diastolic volume like standing up etc....whereas sleeping would increase the VR and so Increase the murmur of HOCM..........also drugs which slow down the heart like Beta blockers and CCB's increase the intensity of the murmur........

a patient comes to you as a part of a health checkup...you are an opthalmologist....you do routine eye examination...on doing opthalmoscopy you find optic disc cupping and on doing perimetry scotomas are found to be present........the IOP is found to be 14mm of Hg..what would be your line of treatment and whats ur diagnosis?????
