CMV is a prefered answer.
In this situation it is difficult to differentiate with symptom and signs alone. Heterophil antibody test is positive in about 50% of children, but positivity increases with age up to 90% in EBV infection.
Downy cells positivity is not a specific finding.
To make a decision with these information complex of this case, it seems reasonable for me to diagnose this case be CMV infection.
