Many questions here are immature...no offense.....
It has been written on multiple threads on rxpg itself about the fact that apply at the earliest, in fact on first day.... this should be in mind at the moment u decide about something like general surgery... UNLESS u r optionless and cant do anything to apply at time....
Second, dont go by program ranking as u r an IMG and why do u care about ranking before u start ranking acutally...even if u apply to 100 program, u wont get a double digit interview unless u r very very exceptional and i dont know any such candidate so far....
FRIEDA is the basic idea about existing programs...next step would be to contact program directors invidually and visit their website and exclude programs who do not consider IMGs or do not sponsor visas... the same rule will apply to observership...apply and apply....
u have to put big time effort if u r going for somethin like general surgery and be prepared for tough times ahead...

Dear Dr.Anil ,
My regards to you fr running such an informative thread inspite of your busy schedule.
My query bout MLE :-
I am currently a Psychiatry
resident in India and who wishes to pursue a residency in the US in any of the neurosciences like Neurology,Psychiatry
or Neurointervention.
My q is being a Psychiatry
graduate confers me and advantage vis a vis getting a university based program.
Also do I hv an option of reduction in the tenure of my residency after getting an MD degree, on the discretion of the program director.
Can u be a bit clearer on the subject of opting to apply fr a fellowship as in Psychiatry
and then be eligible fr the board certification.

First, being a Psychiatry
should benefit u in getting Psychiatry
residency...try to do some research and get some observership at universities like john hopkins etc... Psychiatry
residencies are ranked very low here so very easy to get if u r interested as many people are not which u alrady know....psych is something that one should do only if he likes it and then it is a great thing to do... and u also stand a god chance of getting into university based program..
Second, i dont think u can reduce your residency duration as it is a clinical specialty.... one thing u can do is apply during ur psych residency in usa and if u get it then leave india and u will save one year may be but this is not very highly recommended as u will be under tremendous pressure and might compromise ur score...
fellowship route is usually taken for something like radiology or orthopaedics
may be which are very competitive and fellowships far less competitive than residecnies...in psych, u will face far less competition and as far as i know u cant skip residency process and even in radiology u have to do four year fellowship to be board certified....
Also, if u r interested in neuro, u might have to put extra effort...neuro itself is not bvery competitive as in applicants' choices but it make it little more competitive coz there are less positions of it in usa... but still gettable and u might have little edge but i think u will need to do extra efforts like getting LORs, US clinical experience etc etc....

Thanks Dr.Anil for your prompt reply.I agree that Psychiatry
may not be as competitive to get but it makes much more sense than other specialization if one has an inclination.
And Psychiatry
is not offered to IMGs as easily as it is quite a paying branch for US graduates as far as I know.
Again my regards to you for clearing some of my doubts.

i have a question about visa.
what is the best type visa to go for the step2 cs?
some people have gone on b1 and b2. i called the us consulate in chennai and they advised a family or a visiting visa.
family visa is ruled out for me as i have no close relatives in US.
does the visiting visa serve the purpose given that i may also continue with an observership?
OR is there a better option?

First, avoid calling to US consulate for any reason unless urgent....
Second, Observership is far better reason to go to USA than step 2 CS but these days visa rules are not that strict....

It is paying just coz they do work out of their schedule and get more salary....overall, they dont earn more....
I also dont think that is the reason for med students of USA to consider psych... i have not met a single med student so far out of may be 50 i have met who is interested in psych.... but that does not degrade psych...
I might also not be the right person to suggest and keep on looking for ur answer...

Just my 2 cents.... Psychiatry
is not a very high paying branch compared to other branches and it has a very high proportion of IMGs because US grads tend to prefer other branches over psychi. Bottomline is that psychi is relatively less competitive than most other branches and hence easier for IMGs to get.

Do high USMLE
scores really matter as far as getting a residency is concerned. I know it is not that hard getting a score of 94 or 95. But is it justified to put that extra effort and get to 99. And it is very costly too, given that most people who join Kaplan seem to end up with better scores.
Do programs make a difference between those who score 240 and those who score 235 if both have equally good USCE, LORs etc..
