A 30 yrs old multiparous woman is found to have a 5 to 6 cm cyst in one ovary. The most appropriate line of management would be :
One answer only.
a) Laparotomy and cystectomy
b) Laparotomy and unilateral salpingo oophorectomy
c) Laparoscopic aspiration of the cyst
d) Observation to see if it disappears by itself in three months

msot common ovarian cysts r functional and disappear by themselves.
check this text from bmj journal website :
In premenopausal women a wait and see attitude may be adopted for cysts of benign appearance. If spontaneous resolution has not occurred after three to six months needle aspiration may be performed. In case of recurrence or obvious malignancy, surgery becomes necessary. After the menopause the high incidence of cancer precludes a wait and see strategy. A cyst can be evacuated by needle aspiration if it is unilocular, anechogenic, and smaller than 50 mm and if the patient has serum concentrations of CA 125 below 35 UI/ml. In all other cases, or if a recurrence occurs, surgery is indicated.
note: a physiological cyst will not have septations and excrescences, so if these two r present on usg --- u cant go for conservative approach.

answer is d..... in a reproductive age femle with an adnexal mass less than 8 cms.... the protocol is to observe and if it increases in size then operative intervevtion done.

"Functional ovarian cysts include follicular cysts, corpus luteum cysts, and theca lutein cysts. All are benign and usually do not cause symptoms or require surgical management. The most common functional cyst is the
follicular cyst, which is rarely larger than 8 cm. They usually resolve in 4 to 8 weeks with expectant management."

(The accompanying algorithm also mentions that in premenopausal woman, with cystic mass on USG which is less than or equal to 8 cms should be managed by observation +/- suppression treatment for 8 weeks.)
