about spigelian hernia -surgery involves repair of the area of weakness and return of the abdominal contents back into their normal position. We utilize a plastic screen or mesh (made from non-reactive polypropylene, Gortex® or at times both) to safely reinforce the area in repairing the overwhelming majority of incisional hernias (both primary and recurrent hernias). The decision on which type, size and specific manufacturer of mesh to be utilized will be made by your surgeon at the time of surgery based on clinical evaluation of the defect, measurements of the defect and surrounding weakness and tissue integrity. Surgical repair of hernias is called a HERNIORRHAPHY. Since muscle is neither cut nor sewn together under tension, this technique is called a TENSION-FREE method.so guys answer seems to be spigelian hernia

I agree.. ans is most probably transvaginal ultrasound as the q was which is the most useful test. TVUSG will not only confirm ectopic pregnancy by visualisation of empty uterus but also localise the gestation sac.

rasmussens aneurysm is indeed pulmonary artery...

source : indian journal of radiology and imaging.

Rasmussen's aneurysm, named after the Danish physician Fritz Valdemar Rasmussen (1837 - 1877) is the dilatation of a small-medium sized pulmonary artery in a tuberculous cavity. It is a rare phenomenon caused by weakening of the pulmonary artery wall from adjacent cavitary tuberculosis [1].

Ambiguos/controversial/idiotic/typing error Q in <a href="http://www.rxpgonline.com/forum2.html">AIIMS </a>
:
I REALIZED THIS BECAUSE THIS TIME I MADE AN INDEPTH ANALYSIS OF ALL Q & I FOUND EXAM CONDUCTED BY <a href="http://www.rxpgonline.com/forum2.html">AIIMS </a>
IS FAR FROM PERFECT,READ THIS & U WUD UNDERSTAND WHAT AM I TRYING TO SAY

Q.17 . A Pt. with HB-6gm, TLC 1200,PLATELET-60,000, MCV 12FL,what is the DIAGNOSIS??

A. APLASTIC ANEMIA
B.MEGALOBLASTIC ANEMIA
C.PNH
D. MYELOFIBROSIS

actually in none of the disease MCV can be as low as 12,but if v hv to attempt it then only in option D. MYELOFIBROSIS MCV can be decresed.

BUT IT IS POSSIBLE THAT IT MIGHT BE AN TYPING ERROR BY <a href="http://www.rxpgonline.com/forum2.html">AIIMS </a>
icon_evil.gif icon_evil.gif icon_evil.gif ,& IF MCV WAS 112 OR 120 ANS IS SURELY MEGALOBLASTIC ANEMIA.

Qn.5 Only females are affected in?

A) Schies syndrome
B) Hunters syndrome
C) Hurlers syndrome
D) Gauchers disease
Ans is None(I wonder why aims give such Q icon_evil.gif icon_evil.gif )

Females are affected in all 4 of them.

HERE ALSO I SUSPECT A TYPING ERROR
If the Q would have been like this

Variant of Qn.5) Females are rarely affected in?
A) Schies syndrome
B) Hunters syndrome
C) Hurlers syndrome
D) Gauchers disease

Then ans is B) Hunters syndrome

Q2.Steroid receptor has attachment site for all EXCEPT?
A) steroid hormone
B) Transcription enhancer
C) Transcription repressor
D) steroid Hormone responsive element
Ans: none (Harper’s 25th edition pg 534)

Steroid hormone receptor have several functional domain: one site binds the hormone, another binds to specific DNA regions, a third activates (or represses) gene transcription, and a fourth may specify high-affinity binding to other proteins.
According to this all 4 sites are present on hormone. So probably there is problem with the options

BUT IF V HV TO PICK ONE
Most probable answer is (C) Transcription repressor

Q.15For differentiating b/w Insulinoma and SulfonylUrea related hypoglycemia, the test which is useful is?

A) Antibody to Insulin
B) Plasma C-Peptide level
C) Plasma Insulin level
D) Insulin:Glucose ratio

Ans is
None
Diagnosis of sulfonylurea-induced hypoglycemia requires the measurement of the drug in the serum or urine.

AMONGST THE OPTION BEST IS D) Insulin:Glucose ratio

I HV GIVEN THE SUMMARY HERE & HV TRIED TO GIVE EXPLANATION FOR THIS Q IN MY BOOK

I THINK <a href="http://www.rxpgonline.com/forum2.html">AIIMS </a>
IS CARELESS ,& I SERIOUSLY DON'T KNOW WHAT ANS <a href="http://www.rxpgonline.com/forum2.html">AIIMS </a>
WILL TAKE AS CORRECT IN THIS Q.
THEY DON'T UNDERSTAND WHAT IS THE VALUE OF 4 Q FOR A HARDWORKING HONEST STUDENT.PATHETIC
