Regarding second case I think the immediate cause of death is pneumonia, which is a complication of two diseases (cancer and DM ).
Probably in real life we need to put as a main diagnosis two of these diseases and pneumonia as a complication of both of them. Or, depending on the case and stage of cancer or advanced DM we can put one of them as a main disease, and another one below as a concurrent disease. But pneumonia can not be a main diagnosis if we have advanced cancer or DM , otherwise we could miss the most debilitating, serious and statistically significant disease/diagnosis.
