another voter for 5..serum iron and tibc.
reasons:
1. nutritional cause of anemia is to be worked up first.
2. History of no solid foods.

Anemic baby.
There is no information for suggesting RBC destruction or Blood loosing, anemia inducing drugs or toxins, Blood dyscrasias(inadequate production).
According to the information given, the baby has had only milk for her diet until 14Mo. Baby over 6mo will fall short of iron if fed with milk only diet.
Iron deficiency is the exact diagnostic choice for this case.

A 14-month-old girl is brought to your office with a three months history of progressive pallor, irritability and lethargy. Mother has noticed puffiness around the child's eyes and side of the face when she wakes up in the morning. Diet consists of 6 to 8 bottles (40 to 60 oz) of milk a day but does not eat solid food. Her Hgb is 60 g/L, MCV 68 fL, and platelets 310x109/L. Which one of the following tests is most likely to be helpful in the diagnosis of this child's condition?



1) direct and indirect antiglobulin test

2) stool for occult blood

3) hemoglobin electrophoresis, reticulocyte count

4) transferrin receptor

5) serum iron and TIBC *********

She is definitely have iron deficiency because her mother did not supplement the child diet with iron as iron fortified cereals from 4-12 months
Also her MCV is <80 ( Microcytic anemia) typical with Iron Diff. Anemia
