Harrisons 17th ed
Prinzmetal's Variant Angina: Treatment
Nitrates and calcium channel blockers are the main treatments for patients with variant angina. Sublingual or intravenous nitroglycerin often abolishes episodes of variant angina promptly, and long-acting nitrates are useful in preventing recurrences. Calcium antagonists are extremely effective in preventing the coronary artery spasm of variant angina, and they should be prescribed in maximally tolerated doses. Similar efficacy rates have been noted among the various types of calcium antagonists. Prazosin, a selective alpha-adrenoreceptor blocker, has also been found to be of value in some patients, while aspirin may actually increase the severity of ischemic episodes. The response to beta blockers is variable. Coronary revascularization may be helpful in patients with variant angina who also have discrete, proximal fixed obstructive lesions.
Goodman & Gilman's The Pharmacologic Basis of Therapeutics - 11th Ed. (2006)Variant angina. This is uncommon. It occurs at rest and is caused by coronary artery spasm, again usually in association with atheromatous disease. Therapy is with coronary artery vasodilators (e.g. organic nitrates, calcium antagonists).
Basic & Clinical Pharmacology
10th Ed - Katzung - Chapter 12
Nitrates and the calcium channel blockers are effective drugs for relieving and preventing ischemic episodes in patients with variant angina. In approximately 70% of patients treated with nitrates plus calcium channel blockers, angina attacks are completely abolished; in another 20%, marked reduction of frequency of anginal episodes is observed. Prevention of coronary artery spasm (in the presence or absence of fixed atherosclerotic coronary artery lesions) is the principal mechanism for this beneficial response. All presently available calcium channel blockers appear to be equally effective, and the choice of a particular drug should depend on the patient, as indicated above.
Hence the answer is nitrates and calcium channel blockers but these 3 references do not recommend any particular agent over another.
