First thing is your thesis topic. You choose a topic. You will have a hypothesis in mind. You will either try to prove it or disprove it. Why are you doing this study? There may be many things like -
- You are trying to show a new thing
- There is research on this till date, but the evidence is not conclusively proven
- You are trying to show how this hypothesis works in your country/area
- You are trying to formulate some sort of guidelines or atleast some practical applications for your hospital or region
So here underlies why you are doing the study
You need to do a literature search for this for all the existing evidence for or against.
You need to define your place of study, your inclusion criteria and exclusion criteria.
You need to calculate the sample size required (to have a good power) - meet the statistician for sample size calculation. If it is a pilot study, the sample size is small.
You need to define the methods you are going to use and the materials you are using. You need to identify each and every small thing needed here. Like if you are collecting blood, what all precautions you are going to take, at what time are you going to collect (if this is specific), which vial will you collect the sample in, what reagent do you use to store the sample, how do you transport the sample etc etc and most imp of all is the quality control.
Then comes how you are going to enter the data, what all particulars that you are going to collect, who all will be involved in doing this.
How are you going to analyze the data? What things in your data will you hold to compare/analyze in view of your hypothesis? What are the appropriate statistical methods that can be used?
What is the time frame for the study?
It is good if you have ethical clearance.

There is a lot of literature coming up on this issue. Still a lot controversial remaining about this thing. I think there is a lot of ping pong probably due to the market forces. So is your thesis being supported by some company? Anyway all the best cuz for a thesis in internal medicine this is too good a topic to get to do in india, may be for cardiology superspeciality residents yes, but definitely not for internal medicine residents even in the elite institutes in india....you are lucky indeed, make the best use of it, cuz if you are interested in cardio, this will help you a lot in your career.
