From the homepage of University of Toronto. All hospitals not listed here (in Toronto) you would have to contact directly. But unless you know anybody there, it will be hard. Try maybe to get a family physician who is affiliated with a hospital and can recommend you there... Good Luck!
9. Can I arrange an Observership?
Observerships have no University of Toronto status. Medical practitioners who wish to arrange an observership do so by contacting the hospital where they wish to have observer status.
Persons interested in an observership at Sunnybrook Health Science Centre (www.sunnybrook.ca) must apply directly to the departmental office of the specialty in which they wish to have observer status. The Credentials Office issues observer credentials at the request of Sunnybrook's departmental offices.
At the Hospital for Sick Children (www.sickkids.ca), persons interested must apply directly to the departmental office of the specialty in which they wish to have observer status.
The Medical Education Office at Mount Sinai Hospital (www.mtsinai.on.ca) indicated that observerships there are not longer than one month and must originate with a request from a staff physician. Prospective observers would need to apply directly to the departmental office of the specialty in which they wish to have observer status.
At St. Michael's Hospital (www.stmichaelshospital.com) and the New Women’s College Hospital (www.womenscollegehospital.ca , prospective observers must arrange their observership through a staff physician at the Hospital, who will notify the Medical Education Office.
At the University Health Network (www.uhn.ca/index.asp), those who wish to arrange observerships must do so through the appropriate departmental office. The departmental office will inform the University Health Network Medical Affairs Office. The UHN Medical Education Office is not involved with the credentialing of observers.

Thanks Herrdoktor2,
You have guided me in such a detail of almost all of the hospitals, but what about Credit Valley Hospital, which is close to my house?
Do,I need to talk directly to Head of Department or through Human resource centre?
Thanks in anticipation!

Oh, you are welcome, I just copied it from the UofT homepage.
I don't know about any hospitals not mentioned in that list. Probably find out if it is part of the University Health Network and if yes, follow their directions.
Otherwise it would be best to call and ask who to talk to and introduce yourself. Then send a good CV and Cover Letter and References or whatever speaks for you. But I think making yourself personally known would be key. Most head of departments will probably just ignore emails of people they can't put a face to.
Good Luck!

No problem...
I can't help you with the Physicians Assistent thing, though. I am fresh out of med school, so it is not an option for me and I did not do any research.
Good Luck!
