did the iol go down along with the capsular bag?
cos u r saying zonular dialysis..
then preop it should hav been subluxated lens with zonular loss or pxf..then u should hav used ctr to stretch it and keep it there..or sutures for the bag(i havent seen suturing bag myself other than in videos in seminars,but it is done)


or during surgery itself,lots of pressure was exerted and caused zonular loss...usually u can make out while during surgery that u r damaging zonules..

in this case,anyway u have to send patient to vr surgeon for removal of iol and proper vitrectomy..then put scleral fixation iol

No the IOL was visible at about 5'o clock(in the well dilated pupil) but not in the pupillary area(undilated).....if I had tried to remove it probably would have had vit loss!
any guesses on the next step...?? A decision u may not agree but it worked.....patient has 6/18 unaided now

Changing the A scan reading from 2.5 to 25 by a senior......and junior getting fired because he/she did not interfere....!!!!!!
My prof had to admit...my senior had goofed.

Dilating a post op ACIOL pt....luckily my colleague corrected my blunder.

What to do when the pupil has got smaller and there is a big piece of nucleus in the bag. You have once pinched the iris and its coming time and again to the probe. Working with Appa CV I in phaco 2.
How to save the PC now??
