i just started clinical attachment 2nd day but dont know what to do and my consultant dont know what to do aswell ! , just looking around strangely in ward and hanging around with sho,is any advice from seniors attachees what else or how to starts things up.thanks

you should ask your consultant to give time table for your clinical attachment. I think almost all consultants who offer attachment chances are willingly to give such time table. I fully understand your miseries as I had such miseries in my attachment periods.

U can ask ur consultant to give u some patients to follow up or if u find any patient interesting then u can do that urself. But do introduce urself who u r.
U can also ask ur consultant to give u an audit or research project to work on. Then u can also attend clinics.

Regarding attachment -- the main purpose is to know the system of NHS and try to secure a job.
For that you have to be-
1) proactive and stick to your consultant, show him in each and every opportunity how good you are in your skills.
2) maintain a log book of your learning curve eg- every day the number of patients you have clerked/reviewed, the new information you have gathered or learnt, the courses you have attended, the teaching programmes you have attended etc.
3) have the invt results of your cons 's patients in hand.
4) know all his patients in and out.
5) request him to provide you with an audit topic and do an audit.
6) get involved in poster presentation/case presentations.
7) try to get involved in publications.
8) work hard so that they feel the difference when you are not around.

These are all what I have done and paid me good returns.

Regarding Audit and research---- audit is observation of the present practice in comparison with local or national guidelines. Research is working on a hypothesis to find new knowledge.
Research needs ethical committee permission and funds but audit can be done with local audit committee permission. Research is an one off study where as audit is a cyclical process in which you observe the practices, compare with standards, provide plans for improvement of services and re-audit to find whether there has been any change in the practice after implementation of your plans for improvement.
