can any one please tell me of how to proceed in history taking of costipation of 80yr old lady who has not passed stool for 5 days& tummy is tense& she has osteoporosis& collapsed T8 VERTEBR.she is on dihydrocodeine? icon_smile.gif

i think there is nothing left to ask in history icon_confused.gif

start with introduction, ask if it is ok if u talk to her now. does she need anything to relieve her pain.

then ask her ODP of complaints.
even if u are told that her c/c is constipation, let her say what concerns her first and first evaluate that c/o then go to constipation

details of constipation:
since when? does she have pain before or after opening bowels?
had previous similar c/o?
anything that brings abt the symptoms?

details of pain if any

when did she notice her tummy was tense?
did she take anything to relieve it?

then comes past h/o, f/h, medicine h/o- since when is she takin dihydrocodeine? how much is the dose? what is the route?

if it wre counselling, we could have explained it to her and suggested high fibre diet and laxatives etc.
or even drug route chang, like PCA etc

Hi that answers is perfect but should we ask for history of vomiting with distention and any fever.Breathing difficulty.Tht will decide how fast we have to act.If she has breathing difficulty then mangement wil difer other than just high fibre but they are basically wnat us to elicit the history of dihydoro coedine and t=its side effects.
Coud you please tell me if you have any idea of where the posting by seven sisters is there in this site?They hade given some wondeful material and someone has managed to delete it.Iamanot able to find it again bye
