well,right now,the midline incision after excising the previous scar seems the most logical. icon_smile.gif

but,sum brains are still in favor of using a paramedian incision,as its the most preferred incison in any elective surgery & also the underlying adhesions cud be dealt more efficiently! icon_eek.gif

Dear Members,
The difference in approach lies in the icon_exclaim.gif INDICATION for surgery. During our residency days, we used to open the abdomen via same indcision if the indication was revision surgery or incisional hernia. However, if the indication is adhesive intestinal obstruction, it is wiser to open the abdomen via a paramedian incision as it will allow u to actually see 8) the adhesions to the midline scar and so avoid incising the gut while opening the abdomen. The ecception to this approach is when the patient has an incisional hernia in addition to midline adhesions in which case we have to approach via the same incision and this icon_rolleyes.gif is a difficult surgery for anyone because it involves meticulous and gentle dissection. through distorted Anatomy .
