Indications for ICDs:

1) Secondary prevention:

- arrest seconadry to VF
- sustained VT associated with syncope and significant haemodynamic compromise
- sustained VT with EF<35% but NYHA<III

2) Primary prevention:

- non-sustained VT on 24hr ECG
- inducible VT on EPS with EF<35% but NYHA<III
- familial sudden cardiac death: long QT, HOCM, Brugada, arrythmogenic RV dysplasia secondary to Fallot's repair

ICDs not for:

- spontaneous VT with minimal symptoms and EF>35%
- inducible VT on EPS with EF>35%.


If syncope of unknown origin but haemodynamically significant, sustained VF/VT on EPS + EF<35% - these patients should NOT be offered an ICD.

P.S. have you ever seen an ICD misfiring???

Killip mortality:

I 6%
II 30%
III 40%
IV 80-90%

(that is inital inpatient mortality)

MRC dyspnoea scale:

1) Not troubled by SOB expect on strenuous exercise
2) SOB when hurrying/walking up slight hill
3) Walks slower than peers on level ground, stops due to SOB when walking at own pace
4) Stops to ctach breath after <100m or after a few minutes on level ground
5) Housebound, SOB with ADLs..

I know that we have to ask the patient (if competent) about DNR, but I think that significant functional limitation with ADLs (including being housebound) and one major system or two minor system pathologies are just as important.
