i think actually you all have misunderstood me i am not saying that can not occur in ca. when ever ther is stricture due to carcinoma so very small amount of barium can be passed. and this can be refred as rat tail appearance.

but when ever we see rat tail we have to difrentialte between achalaisa and carcinoma. actually it occrs in both along with rat tail other features are also seen.
like shouldrein in carcinoma and dilatation in achalasia.

and remember in achalasia generalised dilation never occurs it is dilatation of lower segment of oesophagus. whole oesophagus is not dilated.

so rat tail definately occurs in achalsia and can occur in carcinoma with stricure formation and significan narrowing in esophageal lumen. but answer here is definately rat tail.seebailey 22nd edition page 658 it is dilatation of lower segment and not generalised dilatation.
