treatment of choice for akathisia:
pretty simple frnd..
1.reduce the dose of anti psychotic and try B blockers[propranolol]
2.Benzo diazepines
3.Anti cholinergics

The most effective treatments appear to be lipophilic beta-blockers, especially propranolol and metoprolol. Anticholinergic agent use, commonly given to prevent acute dystonias especially in high-risk patients, is controversial as some feel long-term prophylaxis increases the risk of tardive movement disorders. Propranolol should be the drug of first choice.

Yes, propranolol is the DOC for akathisia...

But it may be mistaken for exacerbation of psychosis...

To differentiate b/w the two: give i.m. 2 mg of Benztropine, akathisia will respond while exacerbation of psychosis will not...
