most common cause of csf otorrhoea is head injury which is frequently associated with increase in ICP.as there is further incr in ICP during heartbeat csf otorrhoea is pulsatile.INSTEAD i am disagree with u guys about asom bcoz lighthouse sign is seen if asom is associated with mastoiditis

i dont understand why so many people kept answering the same question once it had already been answered. anyway i am sure it was much appreciated. i only came to this posting because i saw so many posts and i thought there must be an important discussion going on - and i hoped to gain by it. instead i see the same answered stated by many people. anyway, much appreciated i am sure.

iyes ans is asom but i dont think the reason is because of heart beat
i think the actual reason is just thin of a bottle with full of water in it and make its open end to face down then the water comes out in a pulsatile mannner because the space that was lost by loss of water has to be occupied by air thik that bottle is ear canal and the open end is external meatus

Congenital malformations of the inner ear are well described, though the combination with cerebrospinal fluid (CSF) leaks remains controversial. In this paper a case of a bilateral Mondini malformation with a CSF otorrhea on one side is reported. The malformed stapes contains a perforation in the middle of the footplate and associated thinning analogous to a pothole in a mountain stream. The histological findings support the hypothesis of pulsatile flow of CSF as origin of the perforation of the footplate.
