a lady got inserted Cu-T 3 months back, presents with 8 weeks pregancy. most suitable treatment would be
a. remove Cu T and terminate pregnancy
b. remonve Cu T and let pregnancy continue
c. terminate pregnancy and put a new Cu T in place
d. let the pregnancy continue and remove cu T at the time of delivary

with ref plz

cu t with pregancy -remove cu t & terminate the pregancy because with cu t insitu there are high chances of premature labour etc

IIf she is a primi ..then y shud she go 4 loss....Cu-T will come out e cervical dilatation.its removal may interfere e embryo implanted in gravid uterus.i go 4 option 4..continue it till term..

got the ans in park 16th ed. pg 336

it states that the earlier teaching that pregnancy with an IUCD in situ in not unsafe is no longer accepted (too many -ve statements na). anyway that means that pregnancy with IUCD should be regarded as a potential medical complicatio with dangers of infection and spontaneous aboution.
the options are
a) if the woman wants an abortion, it is available.
b)if she wants to continue pregancy and treads are visible then pull the IUCD out and continue pregnancy
c) if threads not visible, a watch for complication should be done

now this patient had an IUCD 3 months back, surely to postpone her pregnancy. so i think the pregnancy should be terminated and a new IUCD placed, ie option B

wat about option --d?we had a patient when i was doin my gyne house job---she had a suspision that her cut is not removed completely (by some LHV in periphery)n she was pergnat too----got prgnant after four years ---we did her x -ray n ther was a piece of cuT in uterus(high up)-----she didnt want to terminate her pregnancy.after consultin senior -we decided not to terminate the pregnancy--and advised her close follow ups and did her c -section when she was full term and removed the cuT at that time---so it was an interesting case and i think if the thread is visible we shud remove it n let the pregnancy continue.otherwise pregnancy can continue with cuT.wat do u think?reference is practical experience:)

when a pt comes with cu t n pregnancy, we generally remove the cu t and ask the pt whether she wants to continue the pregnancy or not.. if she wants to continue , then we allow her to do so but with regular usg moniterring, iucd is not generally removed bcoz it causes teratogenic effects , but bcoz there are high chances of infection with iucd in situ,in other case if cu t thread is not visible, we customarily terminate the pregnancy....... rusulbhai, aapke madat ke zarurat hai icon_redface.gif
